Home palliative care is coordinated by the CLSC in your area, which is responsible for medical and nursing follow-up. Alliance Santé works in partnership with these teams: we provide daily presence and practical support, while the CLSC care team manages the medical aspects.
This collaboration ensures smooth continuity of care, with each provider playing their role for the wellbeing of the client and their family.
